Looks pretty good! The box you're using as a notice box is supposed to be to describe certain things about your character, though, like what their birthday is, what they'd do in certain situations, a little about their personality, and small or major changes to their appearance when they change into another form or alter ego that can be described in a few words, like their fur/hair/eyes changing colors or gaining wings, a tail, marks on them, etcetera, not their full appearance in another form.
